BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/bcr-2014205181 DESCRIPTION A 24-year-old female patient was admitted with symptoms of right lower limb swelling, shortness of breath and dry cough of 1 week duration. There was no history of fever, chest pain, oral contraceptive use, smoking or underlying malignancy. On examination, heart rate was 100 beats per minute, respiratory rate was 24 breaths per minute while other vital signs and systemic examinations were normal except for swelling in the right lower limb. Oxygen saturation on room air was 94%. ECG showed sinus tachycardia. Lower limb ultrasound Doppler examination confirmed right lower limb deep venous thrombosis (right proximal femoral vein with extension into the right superficial femoral vein). Transthoracic echocardiography was normal. Chest radiograph showed a peripheral subpleural wedgeshaped opacity in the right midzone. CT pulmonary angiography (figure 1) demonstrated thrombus in right main pulmonary artery branches with features of ‘reversed halo sign’ (RHS) in the wedge-shaped opacity confirming the diagnosis of distal pulmonary infarction. The patient was treated with a therapeutic dose of low-molecular-weight heparin, improved and was discharged on oral anticoagulants. RHS denotes a focal, rounded area of groundglass opacity surrounded by a complete or nearly complete ring of consolidation on high-resolution CT thorax examination as was seen in our patient. Previously considered to be specific for cryptogenic organising pneumonia, the sign has been described in a wide variety of infectious and non-infectious conditions such as pulmonary fungal infections, pulmonary infarction, granulomatosis with polyangitis, tuberculosis, sarcoidosis, pneumocystis pneumonia, etc. In our patient, presence of thrombus in the pulmonary artery branches with RHS in the pulmonary opacity clinched the diagnosis. RHS, although a non-specific sign, can help in narrowing down the list of differential diagnosis. Final diagnosis should be made in conjunction with all the other ancillary investigations.

Prognostic Impact of Thrombolysis in Myocardial Infarction Risk Index on Hospitalization Mortality of Patient with Acute Pulmonary Embolism
Introduction: Acute pulmonary embolism (PE) is one of the deadly cardiovascular diseases. One of the indexes proposed in these patients for risk stratification is the Thrombolysis in Myocardial Infarction (TIMI) risk index (TRI), which includes three parameters of systolic blood pressure, age, and heart rate. This study aimed to evaluate the predictive value of TRI on in-hospit...
متن کاملRegional right ventricular dysfunction in acute pulmonary embolism and right ventricular infarction.
BACKGROUND A normally contracting right ventricular apex associated to a severe hypokinesia of the mid-free wall ('McConnell sign') has been considered a distinct echocardiographic pattern of acute pulmonary embolism. OBJECTIVE To evaluate the clinical utility of the 'McConnell sign' in the bedside diagnostic work-up of patients presenting to the Emergency Department with an acute right ventr...
متن کاملReversed Halo Sign: Presents in Different Pulmonary Diseases
OBJECTIVE To observe the incidence of reversed halo sign in different pulmonary diseases and the pathological correspondence of reversed halo sign. METHODS Retrospectively studied the high resolution computer tomography scans of all the patients who were admitted in our department with abnormal pulmonary imaging, from 1st of January 2011 to 31st of December 2013, and all the cases with revers...
متن کاملThe diagnostic value of halo and reversed halo signs for invasive mold infections in compromised hosts.
The halo sign is a CT finding of ground-glass opacity surrounding a pulmonary nodule or mass. The reversed halo sign is a focal rounded area of ground-glass opacity surrounded by a crescent or complete ring of consolidation. In severely immunocompromised patients, these signs are highly suggestive of early infection by an angioinvasive fungus. The halo sign and reversed halo sign are most commo...
متن کاملThe rim sign: FDG-PET/CT pattern of pulmonary infarction
OBJECTIVE We aimed to describe a pattern of rim uptake observed in lung infarction on FDG-PET/CT, called the "rim sign." It was defined as a continuous slight FDG uptake along the border of a subpleural consolidation without uptake within the consolidation. METHODS We retrospectively reviewed the FDG-PET/CT studies of 400 patients referred for thoracic oncological workup from November 2010 to...
